You may complete the online nomination form to submit your nomination.

A downloadable version of the nomination form is available in PDF format.

Download and Print or Email (MS Word Format or Rich-Text)
A Microsoft Word and rich-text (.rtf) file format of the nomination form is available for download. You may choose to print the forms and mail or fax them, or email them as an attachment to humanekind@pet-abuse.com.

Deadline
All nominations must be received or postmarked no later than August 1, 2005. The winning recipient will be selected and announced by Oct 1, 2005.

Parent/Guardian Consent
Before any nominee can be considered, we must have consent by a parent or legal guardian. In the nomination form, we require the contact information for a parent or guardian so that we may establish consent. Any entries submitted without a parent or guardian's contact information will not be considered.
